The Living Well program provides resource navigation services to help individuals succeed.
Connection to community resources and programs
Navigating community resources is a challenge. Living Well provides direct connection to resources from basic needs to asset-building. Connect to a Community Service Specialist to learn about:
- Public benefits (SNAP food stamps, AHCCCS medical insurance,)
- Emergency assistance
- Low-cost medical, dental and mental health options
- Housing resources (home rental, fair housing, landlord-tenant issues, etc.)
Well-being and Social Determinants of Health
Financial stress can have a destabilizing effect on health. As such, poverty remains the clearest “social determinant” of health. Health care accounts for only about 20% of a person’s health. If we want to improve health outcomes for vulnerable families, whose health is typically much worse than higher-income individuals, we need to address the root causes of social and economic factors that influence health, such as education, employment, and financial stability. Living Well participants focus on financial well-being through financial coaching, employment and career services and connection to public benefits and community resources.
